Osteoradionecrosis of the temporal bone may produce chronic infection which may result in conductive hearing loss. Conductive hearing impairment due to secretory otitis media following radiotherapy is a consequence well known to radiation oncologists. Published studies show conflicting results, with damage occurring immediately or subsequent to the radiation. Hearing loss may improve or may be permanent. There are a number of variables which confound the effects of radiation, but it has been demonstrated to cause dryness and scaling of the skin of the external auditory canal, leading to build up of debris which may result in conductive hearing loss.

The most common treatment for sudden deafness, especially in cases where the cause is unknown, is corticosteroids. Steroids are used to treat many different disorders and usually work by reducing inflammation, decreasing swelling, and helping the body fight illness. Steroids are usually prescribed in pill form. In recent years, direct injection of steroids behind the eardrum into the middle ear (from here the steroids travel into the inner ear), called intratympanic corticosteroid therapy, has grown in popularity. In 2011, a clinical trial supported by the NIDCD showed that intratympanic steroids were no less effective than oral steroids , but were less comfortable overall for patients. They remain an option for people who can’t take oral steroids.

Depending on timing, intra-tympanic steroid injection is also offered (dexamethasone 10-24mg/cc). Intra-typmpanic steroid injection is performed by inserting a needle through the eardrum and injecting about of highly concentrated steroids directly into the middle ear space. The patient is than instructed to keep the affected ear up for 30 minutes without swallowing, yawning, or popping the ear. After injection, the patient is allowed to immediately resume normal activities. This steroid injection has also been used to treat Meniere's Disease flare-ups.

Clinically, the success rate of Intratympanic steroid therapy in patients with SHL is variable in the literature and the available studies are limited to retrospective and non-controlled prospective ones. In those studies steroids were used in various concentrations, regimens and delivery methods and their effectiveness have not been established due to the lack of randomized controlled trials. There have been some studies in the literature that discussed the effectiveness of Intratympanic steroid therapy as a salvage mode of therapy in patients who failed to respond to oral steroids (Herr & Marzo 2005, Slattery et al 2005).
